# Set up application forms

## What does this do for me?

The **Forms** page publishes the application that customers complete. You either install a starter template that creates everything together, or define a **profile schema** and a **form journey** yourself.

## 1. Open Forms

Open **Forms** in the Customer Gate navigation.

## 2. Start from a production template

The fastest path is **Start from a production template**. Select **Install** on the template. Installing a template creates a validated schema, a published form, an approval rule, and an access rule together. The installed template shows an **Installed** badge.

## 3. Create manually

If you prefer to define things yourself, select **Show manual authoring** to reveal the advanced controls. These accept the raw JSON field and step definitions.

### Create a profile schema

In the **Profile schema** card:

1. Enter a **Name**.
2. Choose a **Target** of `customer` or `company`.
3. Paste the **Profile fields (JSON)**. Each field needs `key`, `label`, `type`, and `required`.
4. Select **Create schema**.

### Create a form journey

A form needs an active profile schema first; without one you see a **Create a profile schema first** warning.

In the **Form journey** card:

1. Enter a **Name** and **Slug**.
2. Choose the **Profile schema** to collect fields from.
3. Choose a **Journey** type.
4. Paste the **Form steps (JSON)**. Leave `[]` to create one Details step.
5. Turn on **Require customers to sign in** if the form must be completed by a signed-in customer.
6. Select **Create form**.

## 4. Review the lists

The **Schemas** table lists each schema's **Name**, **Target**, and **Active** state. The **Forms** table lists each form's **Name**, **Slug**, **Type**, and **Active** state.

## Next step

Once your form is published, review the submissions it collects under **Submissions**. See [Review applications](https://docs.pcnaid.com/nodes/shopify/customer-gate/merchant/guides/review-applications.md).
